Risk: On affected vehicles operating with a software release prior to 2023.38.4 an overvoltage breakdown in the steering rack may overstress motor drive components on the printed circuit board. If the overstress condition occurs while the vehicle is driving steering assistance will not be affected and a visual alert will illuminate. Once the vehicle speed comes to a standstill a loss of steering assistance may occur and loss of steering assistance will persist when the vehicle is driven again. Manual steering without steering assistance remains available to the driver.

Subject: Formal Safety Complaint – Tesla FSD Failure and Missing Video Evidence Vehicle: 2024 Tesla Model Y Long Range VIN: [XXX] FSD Version: FSD (Supervised) v14.3.10 Incident Date: October 9, 2026 Location: [XXX] Dear NHTSA, I am filing a formal safety complaint regarding a suspected failure of Tesla's Full Self-Driving (FSD) system. On XXX, my wife activated FSD while our Tesla Model Y was parked in a parking lot. The vehicle began reversing and collided with another stationary vehicle behind it. Although the other vehicle showed no visible damage, our Tesla suffered significant…

When you disengage full self driving supervised, a nag screen asking you why you turned off Full Self Driving, blocks controls on your screen, and cannot be turned off without the driver being forced to respond. This is dangerous while parking and in other scenarios. Especially since the only way to get rid of it is by being a distracted driver, as you cannot use all controls without first answering the question as to why you decided to take over for the automated driving. This happens multiple times per drive, per day. I have included a picture of the bag screen from another driver as I did…

On September 26, 2026, my 2024 Tesla Model Y (53,000 miles) suffered a total electrical failure. The vehicle became completely unresponsive — it would not power on or enter Drive mode — and my family was locked outside with no way to access the vehicle. Tesla Service Mode diagnostics (run September 29) showed: HVP LV Power Test FAIL with "Contactor power failure detected" — the high-voltage battery contactors would not close. Also present: drive-unit graceful power-off alert (DI_a063), BMS error, and charge-port communication loss (CP_a060). Tesla has issued campaign bulletin SB-24-16-005…

1. i was using tesla self driving standard on my vehicle it accelerated up to 19 miles over the recorded speed limit. 2. The speed limit was 55 and it went to 74 way to high and no way to lower it manually 3. Confirmation by the MD State Police radar 4. No inspection 5. There was no warnings. Consumers should be able to set the maximum speed desired or lower it if it goes to high manually, but you can't if you are using self driving options. You can go to chill which is the next level down but that is to slow. Consumers should be able to manual put the speed where they want to drive and feel…

My 2024 Tesla Model Y (VIN [XXX]) has lost all forward camera-based safety and driver assistance functions. The vehicle displays alerts that Automatic Emergency Braking is unavailable, Park Assist is unavailable, adaptive headlight features are unavailable, and that self-driving is degrading and the vehicle may stop. Autopilot and Full Self-Driving will not engage. The side repeater cameras and the rearview camera still display images normally on the touchscreen, so the failure is not a loss of video display. The loss of Automatic Emergency Braking and forward collision warning means the…

SEVERE DEFECT: Unprompted autonomous steering overrides manual driving. On August 25, 2025, I was in a crowded Trader Joe's parking lot using the vehicle's FSD parking feature. The vehicle was moving backward into a parking position. A stationary vehicle was in the area. Instead of detecting the other vehicle and stopping, the Tesla kept backing up and made contact with the stationary vehicle. The other vehicle was not moving, and there was enough room for the Tesla to park without contact. I do not remember the exact positioning or direction because of the time that has passed. I did not receive a warning that prevented the contact. I reported to Tesla that the…

On June 7, 2026, I was driving westbound on I-290 in the leftmost lane in a 2024 Tesla Model Y. I recall FSD (Supervised) was engaged and making a lane change into the left lane. Instead of settling in the lane, the vehicle continued left and struck the divider/median. I braked. The vehicle then bounced back and moved right toward a large truck and collided. I did not receive a takeover warning before the event. The front bumper came off and the vehicle became inoperable. A fire truck traveling behind me blocked traffic after the crash. To see whether a recall is still open on your car, enter your VIN at nhtsa.gov/recalls (U.S.) or your registration at check-vehicle-recalls.service.gov.uk (UK). Had a safety problem? Report it to NHTSA; complaints are what trigger investigations and recalls.
