My 2017 Ford F-150 is experiencing serious transmission issues that appear to be related to Safety Recall 26S28. The vehicle is shifting harshly, failing to shift properly, shifting only into 3rd and 5th gears, sometimes failing to engage 6th gear, and occasionally losing all power. A generic wrench warning appears on the dash with the message “See Service Manual.” A code reader also displays diagnostic trouble code P0722. I took my vehicle to Great Lakes Ford of Muskegon, where the dealership diagnosed the issue and confirmed that the lead frame needs to be replaced. The lead frame is the component addressed by this recall. However, the dealership stated that because the vehicle was not…

Automatic transmission failed due to code P0722 (Output Speed Sensor signal lost). The vehicle experienced sudden loss of propulsion, violent/harsh shifting, and severe drivability issues while operating on the highway. Loss of forward motive power at high speeds creates an immediate collision hazard. The vehicle was previously serviced for transmission issues but the defect recurred. The vehicle remains parked and inoperable due to safety concerns.

My mother in law contacted me and stated her 2014 ford expedition’s transmission felt like it was failing. I drove same the next day. As I was test driving the vehicle at approximately 60mph down a U.S. highway, I hear a “clunk” noise and then immediately the RPMs went up and the vehicle downshifted into 1st gear causing the vehicles’s wheel to lock up. I went sliding down the highway and luckily didn’t not collide with another vehicle. I had the vehicle scanned and same displayed PO722 output speed sensor codes. After further research, I discovered that her transmission is the same model and design as the 6R80 transmissions that were installed from 2011-2017. The 6R80 transmission lead…

The 2013 Mustang (with 67,071 miles) transmission (6R80) downshifted to 1st gear at highway speeds exactly like the F-150 recall and would also shift out of gear. It could have been catastrophic, but was able to keep control of the vehicle. Was able to get the car home and only drove it to the dealership after this incident. The dash showed the airbag light, no mileage and the wrench similar to the F 150 issue, which pictures are available. Teh vehicle would also sporadically act as if it was in neutral. The vehicle was taken to a Ford dealership and the transmission diagnosis showed codes P0722, P0731. The repair was extremely expensive and should be recalled similar to the F-150 faulty…

I am reporting a recurring transmission defect in my 2019 Chevy Express Cargo Van. Shortly after purchase, I noticed clunking sounds when slowing down or coming to a stop. The issue progressed into rough shifting and eventual check engine lights with codes P0722 and P0700. The dealership stated the valve body had already been replaced in 2020 and suspects the torque converter is the root cause, which is causing repeated valve body failures. These transmission failures create unsafe driving conditions, including loss of smooth deceleration and unpredictable shifting behavior. I believe this is a design flaw affecting the Hydra-Matic transmission system and may qualify as a safety risk. There…

Hello. I recently went in for fords recall regarding 2015-2017 6R80 Transmissions and their limp mode and sudden downshift issues. I got the recall done July 31st (if memory serves) But the fix did not work. I came back in September 3rd to complain and ask them to diagnose and asked if they would fix it this time under the same recall. After 3 hours of waiting they said that my code “P0722-00” does not qualify for lead frame replacement nor did they attempt another fix. I believe this is entirely unprofessional and borderline dangerous. Not everyone can afford a 1200$ repair, especially not for something that should’ve been fixed under the recall. I’ve heard of other potential reports just…

P0722: questions drivers ask

What does code P0722 mean?

P0722 is a generic OBD-II powertrain code. Its standard definition is "Output Speed Sensor Circuit No Signal". The code tells you which system set the fault, not which part failed; the owner reports on this page show what it meant in practice for 8 car models.

Which cars get code P0722 most often?

In NHTSA owner complaints, P0722 is mentioned most for the Ford F-150 (78), Chevrolet Silverado (4), Ford Expedition (3), across 8 models and model years 2012 to 2022. Popular models collect more reports because more of them are on the road.

Is there a recall for code P0722?

No NHTSA recall text names P0722 directly. 8 recalls on the cars that report this code concern the same component owners name most (Power Train); they may or may not relate to the code. Check your VIN at nhtsa.gov/recalls.

What should I do if my car shows P0722?

Write down when the code appeared and what the car did (warning lights, loss of power, noises, speed, weather). Check your VIN for open recalls at nhtsa.gov/recalls, ask a dealer or independent workshop to read the stored freeze-frame data before anyone clears the code, and keep the repair order. If the car loses power in traffic, stop driving it. CarHooq does not diagnose: these are owner reports and official recall notices, not repair advice.

How this page was made

CarHooq read the text of every owner complaint filed with NHTSA for 19 US and European brands (model years 2012 onwards) and picked out the diagnostic trouble codes owners wrote down. This page lists every mention of P0722. The definition is the generic SAE J2012 wording; a carmaker may use the code slightly differently. Nothing here is a diagnosis. Data: NHTSA Office of Defects Investigation (public domain), retrieved November 3, 2026. How CarHooq reports are made.
